Refusal of Medical Treatment

On the October 13, 2005 I reported to the Department of Veteran’s Affairs Eastern Colorado Health Center, Denver Colorado for the performance of Dr. XXXX requested orders for CT-Scan of neck and mid-back area. Please be advised that I refused medical treatment when the above medial treatment orders were changed to performing an MRI on the Thoracic region.
Prescribed Medical Orders:
- As your records show, Dr. XXXX (?) from Denver VA states that I am not to have any MRI’s because of shrapnel imbedded within my body. I do not know the number of times this has been noted in my official medical records, I know of at least two times.
- Dr. XXXX requested the above listed procedures on 23AUg05 and they were changed by a doctor in Denver on the day of my arrival for the test.
Modifying Medical Orders:
- I would like to know why these orders were changed?
- I would like to know the doctor's name of who made these changes.
- I would like to know on who’s authority he/she/… was able to make this change to Dr. XXXX’s orders just prior to trying to give me the MRI.
- It is my understand from doctor’s orders that I was to have a “Contrast” taken during the CT-scan. This was not done. On the prior CT-scan, I was injected with a substance then another scan was done; this procedure was not done on the 13OCT05 test. As was requested.
- I would like to obtain the correct mailing address for Acting Director Berry Sharp
I would like to have the correct mailing address, including name, for the patient advocate, Denver CO. [Done]
Appointment Times:
Side note: I arrived at 1000 hrs for my 1030 hrs appointment. Asked to go to the Ct-scan waiting room. Someone who arrived early for a 1300 hrs appointment was taken in before me. It was then necessary to wait almost another hour before I started my scans! This is just for your information and it’s a matter I intent to bring up with the Patient advocate in Denver. I travel 2 hours each way for my appointments in Denver, arrive on time and then made to wait until after someone who came in early for their appointment. All this is done while I'm on Morphine and other drugs to manage pain!
